What do I have to provide when making an insurance claim against someone?

I was a pedestrian hit by a pickup truck in a private parking lot. No serious injuries but I lost time from work and I'd like to get at least a little something for my pain and suffering. What do I need to provide to that guy's insurance to prove everything?
